take my tears, for its you who made them fall

take my blood, for its you that made it run

take my heart, for its you that made it break

take it all, till theres nothing left to take

take my soul, for without you its empty

take all my dreams, for without you they're nothing

take all the memories, for theyre far too long past

take all my love, till eternity has past

take my emotions, for its for you that I feel

take my pain, for its you that made it real

take my words, for its for you that I cry

take my life, for its for you that I die

[Da Prophecy]

I like that this is dramatic and more directly a dramatic address. It works well and does the emotion in the poem justice. The repetition of the word "take" works well also.
